My hourly rate is available upon contacting my office.

Payment:
I accept cash, check, and most major credit cards.  Payment is due at the time of service.

Insurance:

Reimbursement for services may be covered in full or in part by your health insurance or employee benefit plan.  I am in network with Kaiser, Pacific Source, and I accept out-of-network insurance.  I am happy to help you obtain any reimbursement benefit that you may have. 

Please check your coverage carefully by asking the following questions:

·         Do I have mental health insurance benefits?

·         What is my deductible and has it been met?

·         How many outpatient sessions per year are covered?

·         What is the coverage amount per session?  Do I have a co-payment?

·         Do I have out-of-network benefits? At what percent?

·         What forms do I need to be reimbursed if I use an out-of-network therapist?

·     Do I need prior authorization to see an outpatient therapist?
